The reason of why transcranial doppler should be considered for cryptogenic stroke.
- This test is highly sensitive with close to 100% detection rate for right to left shunt (detection of patent foramen ovale or atrial septal defect) compared to transthoracic echocardiogram which may be less than 50%
- The test is expeditious, non-invasive and safe and done in a clinical setting
- Allows grading of severity of the shunt and predicting risk of stroke
- Able to predict and prognosticate stroke risks in patients who are managed without device closure
